The following CIU and CLI Classes are being offered in the area. Please share with any providers that may be interested.

First, MI and Last Name as they appear on your NYS EMT Card
Your NYS instructor number (for CIU)
Your NYS EMT Number
Full Street Address
Primary EMS Agency Code
Secondary EMS Agency Code (if applicable)
Payment will be collected at the beginning of the course. Only checks may be accepted. Checks should be made out to the “Monroe-Livingston Regional EMS Council”.
