EMS Agency Leaders,
Please take a few minutes to answer the two survey links below. The surveys are a quick set of questions for the purpose of polling the EMS agencies across NYS to determine which agencies may be interested in entering into a contract for service with the NYS DOH BEMSATS. Even if your agency is not interested in entering into a contract we ask that you please answer both surveys.
We ask that you respond by 7:00 pm on 12/12/2021 as current mission assignments MAY begin as early as 12/15/2021.

This request is for one location that is a hybrid model where COVID testing and vaccines will be conducted on certain days of the week.
